Type the number or upload a photo or PDF of the permit, whichever is easier. Your Wisconsin Seller's Permit number starts with 456-. To order nicotine and tobacco products, also add your Local Tobacco License. Up to 5.

We use this only to verify your license and set up your wholesale account. Shipping addresses are added after approval.
